Sports 2009

There will be a "Back to Athletics Night" on August 20th. This will be for all parents and athletics wishing to participate in sports this year. At the program there will be a presentation on sportsmanship and an opportunity to view the "It Could Happen to You Video". Parents will also have the opportunity to fill out paperwork and pay fees prior to the start of practices.

All athletes need to have a Form A or Form B physical sheets, risk form, and sportsmanship form into the office prior to the start of practice. Physical forms can be picked up at your physician's office, at the school office, or can be printed from our website. Athletes need to have Form A completed by their physician every other year. Form B can be completed by parents on the other years.

Risk forms should be completed after viewing the "It Could Happen to You Video" and the sportsmanship form needs to be completed after hearing the sportsmanship orientation. All forms are also available on Wachter's Athletics webpage.

Start dates and times vary for each sport so please check your specific sport website for information.

Physical Form A (pdf document)

This form must be completed prior to participation in NDHSAA athletic events. A health history screening and physical examination is required EVERY TWO YEARS, unless the Health History Screening indicates need for more frequent examinations.

Physical Form B (pdf document)
File Size: 60.26 kb

This form must be completed prior to participation in NDHSAA athletic events. Along with a physical examination on file at the school this Health History Screening is required in years alternating the mandated physical examination. The parent or guardian is accountable to provide accurate information. This information will determine the need for a repeat physical.

Both Parents/Guardians and Student-Athletes are required to sign this form to participate in activities at Wachter.
